Summer Session 4 Participation Agreement
PURPOSE OF PROGRAM
The Beyond Access program offers services that are individualized and go above and beyond standard accommodations. The Beyond Access program is designed to assist you in developing comprehensive academic, technological, and social skills while at the University of Connecticut. Due to the varying needs of each student, your program has been customized specifically to match your learning, social, and emotional profile.
SERVICES PROVIDED
Step 1. You have previously been asked to complete an application discussing your learning and social profile.
Step 2. You were then contacted by a Coordinator in Beyond Access to further discuss areas you would like to work on which may have been determined through answering questionnaires and/or talking with your family members.
Step 3. Following completion of these questionnaires, you worked with a Strategy Instructor to create your customized program designed to meet your academic and personal goals.
Step 4. As part of your Beyond Access program, you will participate in Strengths Finder at the beginning of the semester. This information will be used to help you understand your learning profile and how to apply this information to your academics and personal life.
Step 5. You have agreed that once the semester begins, you will meet on a regular basis with your Strategy Instructor to develop a support and accommodation plan, and to monitor its effectiveness. The meetings will be scheduled at the Center for Students with Disabilities or an agreeable location at your convenience.
The benefits of Beyond Access (BA) rely upon your commitment and involvement with the program. Your Strategy Instructor will help you identify specific strategies to help you reach your academic and personal goals; however it is up to you to apply and utilize these strategies. By signing below, you indicate your recognition with the fact that participation in BA is entirely voluntary and does not guarantee a learning increase, a grade increase or any other specific outcome.
TERM and TERMINATION
The Beyond Access program is offered on a semester-by-semester basis and begins on the first day of classes and end on the last day of final exams, as defined in the University of Connecticut Academic Calendar, found here: https://registrar.uconn.edu/academic-calendar/
You or the Center may cancel (terminate) this Agreement prior to the end of the current semester for any reason with one week written notice to the other party. Upon termination by either party, refunds may be granted on a prorated basis up until the fourth week of classes per the Beyond Access Refund Policy. Pursuant to this policy, no refunds will be offered after the fourth week of classes. If you wish to terminate the Agreement, you must send written notice to: christine.wenzel@uconn.edu.
CONFIDENTIALITY
- Your records and personal information are subject to the Family Educational Rights and Privacy Act of 1974 (FERPA) and may not be disclosed without your permission or as provided under the law.
- For example FERPA permits schools to disclose your information in the event of a health or safety emergency as determined by University employees. For more information please refer to https://ferpa.uconn.edu/.
- Any information used for research purposes will in no way include your name/identifying information.
PAYMENT INFORMATION
The fee for Beyond Access will be added to your fee bill. Payment is due in accordance with university deadlines. If payment is not received restrictions will be applied to your PeopleSoft account. If you are eligible for financial aid, the cost of participation in this program will be taken into consideration. ACKNOWLEDGEMENT
Governing Law. This Agreement shall be construed in accordance with and governed by the laws of the State of Connecticut.
Indemnification. The Contractor shall indemnify and hold harmless the State of Connecticut, including any agency or official of the State of Connecticut from, and against all costs, claims, damages, or expenses, including reasonable attorney’s fees, arising from its negligent acts or omissions in connection with the performance of this Agreement.
Nondiscrimination. The parties agree to comply with all applicable state and federal laws, rules, regulations and executive orders governing equal employment opportunity, immigration, nondiscrimination, including the Americans and Disabilities Act, and affirmative action.
